Terms and conditions

1. AUTHORIZATIONS

Work & Materials: I hereby authorize the Class 8 Brake & Air LLC to perform the repair work described above, including the procurement of necessary parts and materials.

Operation & Testing: I grant the Class 8 Brake & Air LLC and its employees permission to operate the vehicle on public streets or elsewhere for the purposes of testing, inspection, and delivery.

Sublet Work: I acknowledge that some specialized repairs may be performed by third-party providers at the Class 8 Brake & Air LLC discretion.

2. LIABILITY & WAIVERS

Loss or Damage: The Company is not responsible for loss or damage to the vehicle or articles left in the vehicle due to fire, theft, accident, or any other cause beyond the shop’s control.

Personal Property: Customer is responsible for removing all personal belongings. The Class 8 Brake and Air LLC is not liable for items left in the vehicle.

3. PAYMENT & LIEN TERMS

Payment in Full: Full payment is required via [Cash/Credit] before the vehicle is released.

Mechanic’s Lien: I acknowledge an express mechanic’s lien on this vehicle to secure the amount of repairs, parts, and storage fees.

Storage Fees: A storage fee of $50.00 per day will begin [3] days after the Customer is notified that repairs are complete.

Collections: If legal action is necessary to recover unpaid balances, the Customer agrees to pay all reasonable attorney fees and court costs.

4. PARTS DISCLOSURE

I decline the return of replaced parts.

No warranties on used parts or customer purchased parts.

Note: Some parts (cores) may require a surcharge if returned to the customer.

5.ESTIMATE AND UNAUTHORIZED CHARGES: The Class 8 Brake and Air LLC shall provide a written estimate for all parts and labor. The Customer agrees to pay the estimated amount plus a margin not to exceed 10% of the original total. If the actual cost of repairs is expected to exceed the estimate by more than 10%, the Class 8 Brake and Air LLC must immediately notify the Customer and obtain written or oral authorization before proceeding with additional work.

6. SIGNATURES

I certify that I am the owner or authorized agent of the owner of the vehicle described above and have the legal right to authorize these repairs.
